Chyllia Dixon, MSN, PMHNP
Chyllia Dixon is a Psychiatric Mental Health Nurse Practitioner and educator based in Phoenix, AZ, who leads Hazelwood Mental Health with compassion, expertise, and a holistic spirit. Since moving into the mental health field nearly two decades ago, Chyllia has developed a unique, evidence-based psychiatric treatment approach that incorporates a deep respect for somatic presence and mindful connection.
As an author and thought-leader, she speaks passionately about bridging healthcare gaps—advocating for patient-centered, small-practice models that empower both clinician and client alike.
A stewardship of legacy guides her work: Hazelwood’s name honors both her late grandmother Hazel Dixon and the practice’s founder, embodying a multi-generational commitment to healing and relational care. Chyllia brings that intention into every session—creating spaces where clients are seen, heard, and supported to sit with their emotions without pressure or performance. Whether teaching in local LEND programs or supporting families through ADHD, grief, or burnout, she continually reminds patients: “Just being is sometimes the most powerful medicine.”
Courtney Nelson, MSN, PMHNP
Courtney Nelson is a certified Psychiatric Mental Health Nurse Practitioner based in Arizona, where she brings a grounded, integrative approach to adult mental health care at Hazelwood Mental Health. With a Master of Science in Nursing and a deep commitment to trauma-informed support, Courtney helps clients navigate anxiety, grief, PTSD, and emotional overwhelm with compassion and clarity.
Trained in Compassionate Bereavement Counseling under Dr. Joanne Cacciatore, Courtney offers a rare depth of presence for those experiencing profound loss. As a certified yoga instructor, she weaves somatic practices, mindfulness, and meditation into her psychiatric care—meeting each client with respect for their body, history, and personal pace.
Courtney’s philosophy aligns beautifully with Hazelwood’s values: structure creates freedom, healing is personal, and safety is essential. Whether guiding a session in silence or with shared insight, she holds space for clients to feel seen, soothed, and steadily supported on their journey.
Tara Mahoney, MSN, FNP, PMHNP
Tara Mahoney is a Psychiatric Mental Health Nurse Practitioner and Family Nurse Practitioner at Hazelwood Mental Health. With almost 20 years in healthcare, Tara brings a warm, integrative approach grounded in the understanding that true healing addresses the interconnectedness of mind, body, and spirit.
Tara focuses primarily on psychiatric and mental health services, offering evidence-based care that is both thoughtful and individualized. She also provides hormone replacement therapy and medical weight loss support, recognizing the important role physiology plays in mood, cognition, and overall well-being. Her approach emphasizes collaboration, curiosity, and respect—creating space for patients to feel supported rather than rushed or reduced to a diagnosis. She strives to create a calm, attuned environment where patients feel safe to explore their concerns and actively participate in their care.
In harmony with Hazelwood’s core values, Tara is deeply committed to restoring humanity to healthcare. She walks alongside clients through every stage and transition, honoring both the science of medicine and the body’s innate wisdom for growth and healing.
