About Services

We offer psychiatric evaluations, medication management, and brief supportive therapy as part of follow-up care. We also have a strong network of referral partners to help connect you with any additional support you may need.

Our providers can offer brief, supportive conversations alongside psychiatric care, but we are not therapists. When deeper therapeutic work is needed, we'll help connect you with the right fit.

We support patients with anxiety, depression, ADHD, bipolar disorder, OCD, trauma, and autism. If you're unsure whether we treat your specific concern, reach out and we'll help you figure it out.

Yes. We see patients ages 5 and up, including children, adolescents, adults, and older adults.

If substance use is an active primary concern, we may not be the best fit. However, if you have a stable history of substance use alongside a psychiatric condition, we're happy to have a conversation about whether Hazelwood is right for you.

We support patients with autism but do not provide formal autism diagnoses, which require specialized testing. We're happy to help connect you with the right resources for that evaluation.
Getting Started

It depends on your insurance plan but generally, no. You can call the number on your insurance card and ask or let us know and we can help you figure it out too.

Your first visit is a comprehensive evaluation where we take the time to get to know you, your history, your current challenges, and your goals. It's a conversation, not an interrogation.

We ask for at least 24 hours notice for changes or cancellations. Late cancellations may incur a fee. We understand life happens, just keep us in the loop.
Appointments and Telehealth

Yes. Most appointments are available via secure, HIPAA-compliant video sessions accessible from any device. We do ask that all new patients come in for their first visit and return in-office annually.

We are appointment-based and do not offer walk-in services. Please reach out first so we can best support you.
Insurance and Payment

Yes. We are in-network with most major plans including Medicare, Aetna, Blue Cross Blue Shield, Cigna, and UHC. There can be carve outs that make this a complex question. The best route is to call us or your insurance plan to verify in-network coverage.

Yes. Self-pay rates are competitive and based on appointment length and type. Initial evals are $350. Follow up visits an range from $125 to $200.

We accept credit and debit cards, HSA and FSA cards. Payment is due at time of service.
Medication and Precriptions

Refill requests can be submitted through our patient portal or by calling the office. Please allow 48 business hours for processing.

Always. We encourage open, honest conversations about how your medication is working. Please reach out to schedule an early appointment if you have questions or concerns.
Crisis and Emergencies

If you are in immediate danger, call 911. For mental health crises, call or text 988 to reach the Suicide and Crisis Lifeline. You can also visit Mind 24/7 for local crisis support. Hazelwood is not a crisis center and cannot guarantee immediate response outside of session hours.

We do not offer after-hours support, but we do have an after-hours line available for urgent matters that cannot wait until the next business day. Please reserve this line for true urgencies. Non-urgent messages received after hours will be handled on the next business day. After-hours calls may be subject to a charge and if not covered by your insurance, you will be billed accordingly. If you are experiencing a mental health emergency, please call 911 or contact the Suicide and Crisis Lifeline by calling or texting 988. Running out of stimulant medication is not considered an after-hours emergency.
